For more than 25 years, Hospice at Home has been dedicated to serving our neighbors throughout Southwest Michigan – the place we proudly call home. Hospice at Home is your local nonprofit, independent hospice, with care teams including physicians, nurses, home health aides, social workers, bereavement and spiritual care coordinators and volunteers. We are experts at end-of-life care and provide quality care to patients and families in need of comfort care and symptom relief. As the largest and oldest community-based palliative care and hospice organization in the area, we provide 24-hour hospice care from one of our three conveniently located offices.(must have life-limiting illness to be eligible). Office hours are from 8 a.m. to 5 p.m. Monday through Friday, with 24 hour on-call availability.

Our non-profit United Way agency has been caring for clients at home, in nursing facilities or at our facility, Rose Arbor Residence, since 1981. We offer grief support services for hospice families and the community. Hours are from 8 a.m. to 5 p.m. Monday through Friday, with 24 on-call availability.

The mission of SouthernCare is to assure that residents of its service locales have access to a program of care that enhances the quality of life for the terminally ill and their families. Care of the whole person is emphasized by providing physical, spiritual and emotional support to all medically appropriate persons, without discrimination. Efforts are centered on meeting needs by offering highly individualized compassionate care in the person’s home or a home like setting. We service 9 counties in Southwest lower Michigan out of our Kalamazoo office.
