Hospice and Palliative Care of St. Lawrence Valley offers full and part time positions with benefits and mileage reimbursement. RN positions, part time and per-diem

Function: Provides palliative nursing care to the terminally ill and their families which emphasizes their physical, psychosocial, emotional and spiritual needs.

Responsible to: Director of Patient Services

Characteristic Duties and Responsibilities:

  • Provides nursing or bedside nursing for hospice patients
  • Functions as relief nurse for hospice primary nurses
  • Implements interventions identified in the plan of care to provide symptom control and quality of life for patients
  • Uses assessment skills to identify problems that may be resolved, diminished or prevented through interdisciplinary interventions
  • Documents assessments, interventions and outcomes in the patient's record and communicates changes with the interdisciplinary team
  • Provides supervision to LPNs who may be providing skilled nursing
  • Participates in staff training and educational programs related to the Compliance Plan and to adhere to the organization's Standards of Conduct
  • Currently licensed to practice as a Registered Professional Nurse

Qualifications
:
  • 2 years experience in acute care setting
  • Valid New York State driver's license

Licensed Practical Nurse (LPN), Full and Part Time openings

Function:  Responsible for carrying out assigned nursing functions. Provides nursing care under the supervision of an RN in accordance with the policies and procedures of Hospice of St. Lawrence Valley, Inc.

Responsible to: Director of Patient Services.

Characteristic Duties and Responsibilities:

  • Follows plan of care as established by the Interdisciplinary Team and under the supervision of a Registered Nurse.
  • Provides direct nursing care  within the scope of LPN practice.
  • Provides education to the patient/family as indicated in the plan of care.
  • Observes and reports changes in pt/family condition to the primary nurse.
  • Provides emotional support to the patient and family throughout the dying process.
  • Documents in a clear, concise, accurate and timely manner
  • Maintains flexibility in assignments to meet agency needs.
  • May provide on-call services under the supervision of a Registered Nurse.

Qualifications:
  • Current New York State LPN license
  • Current New York State drivers license
  • Ability to maintain confidentiality and function as a member of the Interdisciplinary Team.

Home Health Aide, Full and Part Time Openings

Classification: Non-Exempt

Function: Provide quality direct care to hospice patients in their home

Responsible to: Director of Patient Services

Characteristic duties and responsibilities:

  • Follows plan of care as established and supervised by primary nurse.
  • Performs simple health procedures that are taught and monitored by the nurse or therapist.
  • Monitors vital signs and reports to appropriate nurse.
  • Assists patients with activities of daily living, which may include bathing, oral hygiene, hair care, nail care and shaving.
  • Assists with patient transfers.
  • Assists with ambulating.
  • Assists patients in performing exercises that have been prescribed by appropriate professional personnel.
  • Assists patients with self administered medications.
  • Advises nurses of any changes in patient's needs or condition.
  • Attends and contributes to interdisciplinary team meetings when possible.
  • Performs light housekeeping duties and prepares meals
  • Documents activities as required.
  • Performs related duties as indicated.
  • Participates in staff training and educational programs related to the Compliance Plan and to adhere to the organization's Standards of Conduct.

Qualifications:
  • High school graduate
  • Completed Home Health Aide Program approved by New York State Department of Health and are a New York State Certified Home Health aide.
  • Must have working knowledge of basic first aid.
  • Current driver's license.
  • Ability to maintain a ligh level of confidentiality.
  • Basic understanding and commitment to the Hospice concept of care for the terminally ill.
